Services: Adult Day Centers
Hebrew Community Services operates two adult day centers. Both centers have received Certification with Commendations as Medical Model Adult Day Centers. One site is on the Hebrew Health Care campus in West Hartford, and another is at the Immanuel House senior residential housing complex on Woodland Street in Hartford.
The day centers are a home away from home for hundreds of older adults. They are a place to gather with peers and engage in a variety of activities in a safe and active environment. They are also a place for older adults to receive health services necessary to help maintain their well being and quality of life.
As with all Hebrew Community Services programs, our professional staff develops a customized plan around the specific needs of each participant, and revise that plan on a regular basis.
Participants enjoy breakfast, a hot lunch, and refreshments every day. Meals are even available to bring home (we can accommodate special diets). Transportation is available to and from both of our adult day centers. We offer daily exercise classes, social events, shopping trips, and cultural activities such as concerts and theater.
Our health services include nursing assessments, assistance with medications, rehabilitation therapies, medical clinics, nutritional counseling, and help with showers or baths. Our health care staff will also develop a comprehensive care plan to help members remain safely in the community.
The Hebrew Community Services adult day centers are open Sunday through Friday, from 7:30 am to 4:00 pm. Extended hours are available upon request.
Hebrew Community Services provides transportation for each of its adult day centers from points throughout the Greater Hartford area. Adult day center members are picked up between 8:30 and 9:00 in the morning and are dropped off between 3:30 and 4:30 each afternoon. Special arrangements can be made for other times.
